- Production of testosterone in the testes is stimulated by luteinizing hormone (LH or Lutropin) and an androgen deficiency can be caused by hypogonadism, age-related Leydig cell dysfunction, pituitary or thyroid health conditions or medications that may interfere with testosterone production by the testes.
- Healthy levels of the two gonadotropins, luteinizing hormone (LH) and follicle-stimulating hormone (FSH) are key to proper testosterone production in men. LH is also called the interstitial cell-stimulating hormone (ICSH) because it stimulates Leydig cell production of testosterone. A man's brain regulates proper LH output via estradiol levels detected in the body. If estradiol (estrogen) levels are abnormally high, then LH shuts down and testosterone production declines. Conversely, if LH/FSH is too high, a negative feedback response will also shut down testicular activity. In either case, over-secretion or diminished secretion of LH/FSH can result in failure of testicular function.
- Less Free T or Bio-Available Testosterone. The aromatase enzyme that converts testosterone to estradiol is found in adipose fat cells and can reduce the amount of Free Testosterone (Active Testosterone) in men. Men with central adiposity, high cortisol or prolaction levels and/or a DHEA imbalance may have more aromatase enzyme activity and therefore convert more testosterone to estradiol leaving less bio-available or active testosterone in their system. Excess estradiol (estrogen) levels in a man's system competes directly with a man's bio-available or active testosterone by interfering with the male hormonal message the testosterone is trying to deliver to the body's cell response sites.
- Erections and the quality of erections are clearly (androgen) testosterone-dependent. Men with an androgen deficiency or hypogonadism have a reduction in
the frequency and rigidity of erections. During andropause a significant decrease in Free T (Bio-Available or Active Testosterone) occurs as increases in sex hormone-binding globulin (SHBG) concentrations occur. During andropause the total testosterone level declines and the remaining Free T or bio-available testosterone is bound ever more tightly to SHBG creating an even greater androgen - testosterone deficiency.
- Excess Prolactin levels in a man's bloodstream can cause infertility, testicle dysfunction, decreased testosterone levels, abnormal sperm or decreased sperm count. Hypogonadism, where the testes do not produce the right amounts of testosterone can cause decreased sex drive, erectile dysfunction or impotence.
- High Cortisol levels can induce aromatase which speeds up the conversion of testosterone to estradiol resulting in elevated estradiol levels and decreased Free Testosterone (Bio-Available or Active Testosterone) levels.
Viagra normally works if there are sufficient levels of testosterone, therefore Viagra may not be the perfect solution for all sexual problems including impotence, erectile dysfunction (ED) if a man's levels of total and bio-available or active serum tesosterone are significantly low.
